Sports Physicals and COVID-19 information
- Due to the rare incidence of heart complications resulting from a COVID-19 infection (learn more here), New York State now requires that parents/guardians of high school student-athletes complete and sign a student-athlete health history form that includes questions about the student's COVID-19 and family heart health history. Also, any athlete who had COVID-19 must have a physician's release, clearing the student-athlete for competition.
- To be eligible to practice and compete, parents or guardians must provide the following to the school nurse before the student-athlete’s first practice:
- The student-athlete Blue Card,
- COVID-19 and Family Heart Health history form,
- Proof of a current physical,
- Physician's release to compete after COVID-19, if needed.
- Return to play - If a student tests positive, Central Valley will follow this Return to Play for athletes and PE protocol.
